A.  Applying for New Service Connection

About the Service:   Who may avail of the service?
New service connection will not be made
until it is approved and charges are paid.
Installation of service connection shall be on
first come first serve basis.

  All residents/establishments within MWD service area. 
Schedule of Availability of the Service:   What are the Requirements?
Monday-Friday
8:00 am – 5:00 pm
No noon break

  Accomplished application form Mayor's permit for water connection, Residence certificate Payment of NSC fees.
Duration:    
Application, 30 minutes to 2 hours depending on the complexity. Survey and Service Line Tapping is within 15working days under normal circumstances.    

B.  Payment of Water Bill

About the service:   Who may avail of the service?
A penalty charge is added to bills paid after
due date. Services will be discontinued without
further notice if payment of the bills is not made
by due date indicated in the water bill notice.

  All concessionaires of Muñoz Water District.

Duration:
5 minutes under normal circumstances

  Water bill receipt/Account information
Cash/check payment
C.  Reconnection of Service    
     
About the service:   Who may avail of the service?
Reconnection Fee must be paid before reconnection
of disconnected service connection. 

Disconnected service for one year whether
delinquent or requested shall not be considered
reconnection but instead concessionaire shall
apply for new service connection.


  All concessionaires of Muñoz Water
District with disconnected service con-
nection. 

Duration:
Within 3 days under normal circums-
tances.

  Payment of water bills, fees & charges
     
D.  Relocation of Water Meter     
     
About the service:   Who may avail of the service?
Relocation of water meter from one place to
another. Extension of stand-pipe is considered
relocation.

  All concessionaires of Muñoz Water 
District.

Monday - Friday
8:00 am – 5:00 pm
No noon break




  Accomplished service request form
and payment of all necessary charges

Duration:
Within 10 days under normal circums-
tances.

E. Attend Service Requests and Complaints  
     
About the service:   Who may avail of the service?
Receives all service requests/complaints
at the MWD office, personal or thru phone
from concessionaires, indicating the nature
of complaints and service defects

Services Available:
  Change gate valve
  Temporary Disconnection
  Leak Repair
  Dirty water
  Low/High Consumption
  Broken/check meter
  All concessionaires of Muñoz Water
District.



Duration:
Within 1 working day under normal
circumstances.
